NEXT STEPS

You will shortly get a phonecall from the diabetes care team in Homewell Practice to arrange for you to come into the practice to have your bloods & physical measurements taken and be given advice around medication management.

Once you have completed this review with your Homewell doctor you will be granted access to the platform to start your treatment journey.
