RediCare Control is a digital therapeutic and supporting clinical service that treats patients with chronic conditions. The service is designed to quantifiably show reduced patient demand in your primary care network. We do this by improving your patient's metabolic health, reporting these outcomes and the associated improved PAM, GAD2 and PHQ2 markers. The service is delivered within the Enhanced Access Hours.
About Demand Reduction
We believe we can deliver demand reductions in your PCN based on clinical outcomes and resulting improved PAM, GAD2 and PHQ2 markers. Recent results from deployments in primary care networks like yours have shown an average reduction in HbA1c of 14.7 mmols/mol (-23.4 %) and body weight reduction of 5.3 kg (5.3%).
Studies have shown that these types of improvements in patients metabolic health and associated improved PAM scores can lead to approximately 5 less doctors visit per annum, per patient.
Deploying RediCare Control in your Primary Care Network
RediCare Control manages the entire service delivery. Your PCN's only task is to clinically approve each patient cohort, we manage the rest. We message the patients to onboard, deliver the treatment, update your EMIS records, report regularly on your patient's clinical outcomes and projected demand reduction.
